What to bring:

• A list of your medications
• A list of prior surgeries
• A list of any illnesses
• Any medical records, labs, x-rays, or referral letter from
  your primary care doctor
• Your insurance cards
• Your co-payment amount

Please arrive a few minutes early for a first appointment as we will have a little paper work to complete.
